Different signs of facial aging respond to different treatments. PRF is commonly used for under-eye hollowing where subtle, regenerative improvement is preferred over added volume. works throughout the face and neck to stimulate collagen and improve skin quality, adding subtle volume and thickening the skin over time. Radiesse is more versatile: in its standard form, it acts as a filler to rebuild structure and volume in the face, and in a hyperdilute form it restores collagen and thickens the skin across the face or neck. Of these collagen-stimulating options, Radiesse has the most capacity to address jowling.
There is no single “best” option. The right choice depends on where volume loss, skin thinning, or laxity is occurring, and how quickly results are needed.
The under-eye area is one of the most delicate treatment zones in facial aesthetics. Skin is thin, movement is constant, and small changes in fluid balance are easy to see.
Hyaluronic acid filler adds immediate volume to reduce shadowing under the eyes. It can be very effective when placed precisely. Results are visible right away, which is one of its main advantages.
In the under-eye area, though, filler requires careful selection and technique. This region is more prone to swelling or puffiness in some patients over time.
PRF is created from a small blood draw and processed to concentrate platelets, fibrin, and growth factors. Instead of adding volume, it works by encouraging gradual tissue improvement.
Over several weeks, PRF supports collagen production and subtle thickening of the under-eye skin. This can soften hollowing and improve overall skin quality in a more gradual way.
At Vitality Aesthetics, PRF is used specifically for under-eye rejuvenation and hair restoration.
The table below compares both options across the factors that matter most for under-eye treatment specifically.
| PRF Under-Eye Injection | Traditional Hyaluronic Acid Filler (Under-Eye) | |
| How It Works | Stimulates collagen using your own growth factors | Adds immediate volume |
| Results Timeline | Gradual; improvement develops over 4 to 8 weeks as collagen remodeling progresses | Immediate; volume is visible from the day of treatment |
| Longevity | Results commonly last 6 to 12 months; repeat sessions build on previous results | Typically 12 months or longer, sometimes lasting years; filler often persists in the under-eye area longer than in other zones |
| Ideal Candidate | Patients with mild to moderate hollowing, thin or sensitive under-eye skin, or those who prefer a regenerative approach over added volume | Patients with true hollowing (rather than under-eye “bags”) who want immediate correction. Note that the cheek and temple area is typically corrected first, since volume loss there often contributes to under-eye shadowing |
| Downtime | Mild bruising and swelling common for 3 to 5 days due to the delicacy of the under-eye area | Similar; bruising and swelling for 3 to 7 days, occasionally longer |
The comparison table highlights the key differences, but the best option depends on your anatomy, skin quality, and the degree of volume loss. It’s also worth knowing that under-eye shadowing is often driven by volume loss in the cheeks and temples, so we usually address those areas first and treat the under-eye directly only if it’s still needed. This “treat the root of the problem” approach tends to produce the most natural-looking result. During a consultation, we’ll assess your under-eye area, talk through your goals, and recommend the treatment most likely to deliver natural-looking results.
Jowling is one of the structural changes that patients find most distressing, in part because it tends to happen gradually and then seems sudden. The lower face begins to lose the scaffolding (fat pads, collagen, bone density) that kept the jawline defined. Soft tissue descends. The transition from cheek to jaw softens, eventually creating the jowl pattern that many women associate with looking older than they feel.
Sculptra is a collagen-stimulating injectable made from poly-L-lactic acid (PLLA), and it’s one of our favorite tools, though not for jowling specifically. Think of Sculptra as fertilizer for your skin: it encourages your body to gradually build new collagen, which improves skin quality, softens fine lines (including the ones that show when you smile), adds a natural glow, and creates subtle volume and thickening across the face and neck.
What Sculptra doesn’t do is deliver the structural lift that jowling calls for. When the lower face needs real support, we reach for a hyaluronic acid filler or, more preferably, Radiesse. Radiesse works as both a filler and a collagen biostimulator, and of these treatments it has the most capacity to rebuild lower-face structure and soften jowling. During your consultation, we’ll identify what’s actually driving the change in your lower face and match the treatment to it.
The neck is one of the earliest areas to show changes in skin quality, often before the face shows significant aging.
Hyperdilute Radiesse uses calcium hydroxylapatite (CaHA) in a heavily diluted form. Rather than adding filler-style volume, it spreads across the tissue to stimulate collagen and improve skin firmness over time. At Vitality Aesthetics, we use it across the entire face and neck to improve skin texture, soften fine lines, firm and thicken the skin, define the jawline, and provide gentle lift and support. On the neck specifically, patients often notice gradual improvement in crepey texture, horizontal neck lines, and overall tightness in the weeks following treatment. When more pronounced structural volume is the goal, standard (non-diluted) Radiesse is used instead.
This is not a neck lift. It does not address excess skin or significant ptosis. For patients with mild to moderate neck laxity and crepey skin quality who want a meaningful non-surgical improvement, it is one of the most clinically targeted options available.
Changes in the under-eyes, jawline, and neck often develop at the same time, even if one area stands out more than the others.
That’s why it’s common for patients to end up with a layered approach instead of a single treatment.
For example:
Each treatment works at a different depth and on a different aspect of aging, which is why they can complement each other when used thoughtfully.
What matters most is not combining treatments for their own sake, but matching them to what’s actually happening in your anatomy. That becomes clear during a consultation, where timing, priority, and sequencing are mapped out based on your goals and facial structure.
One thing patients consistently underestimate is how much the results of any regenerative treatment, whether PRF, Sculptra, or Hyperdilute Radiesse, depend on their internal biology. These treatments work by triggering your body’s own collagen and regenerative response. How robust that response is depends heavily on your hydration, nutrition, inflammatory load, sleep quality, and hormonal environment.
At Vitality Aesthetics, every treatment is viewed through the lens of the VClear Method™. In-office procedures are paired with a focus on the internal factors that influence skin health, including hydration, nutrition, and overall wellness. Because collagen production is a biological process, supporting those foundations can help patients get the most from regenerative treatments over time.
This is the difference between a treatment and a treatment plan.
The best treatment starts with identifying what’s causing the change you’re seeing. During your consultation, we will evaluate your facial anatomy, discuss your concerns, and determine whether PRF, filler, Sculptra, Hyperdilute Radiesse, or a combination of treatments is the best fit for your goals.
If you’re looking for a non-surgical approach to under-eye hollows, early jowls, or neck laxity in Haymarket or Northern Virginia, schedule a consultation with Vitality Aesthetics. We’ll explain your options, answer your questions, and create a personalized treatment plan designed to help you achieve natural-looking, long-lasting results.

Neither is universally better.
The right choice depends on your anatomy, the degree of hollowing, and your history with under-eye treatments. PRF is derived from your own blood, carries no migration risk, and works by stimulating your body’s own collagen response over four to eight weeks. Traditional hyaluronic acid filler delivers immediate volume and may be the stronger option for patients with more significant hollowing who need faster correction. An in-person assessment with your provider is the only reliable way to determine which is appropriate for your under-eye specifically.
Not in the way many people expect. Sculptra stimulates collagen to improve skin quality, soften fine lines, and add subtle volume and thickness, but it doesn’t provide the structural support that jowling requires. When jowling is the concern, we typically use a hyaluronic acid filler or Radiesse, which works as both a filler and a collagen biostimulator and has the most capacity of these treatments to rebuild lower-face structure. During your consultation, we’ll look at what’s driving the change and recommend the option best suited to your anatomy.
Hyperdilute Radiesse uses calcium hydroxylapatite (CaHA) that has been significantly diluted and distributed across a broader area of the neck.
This technique creates a supportive framework beneath the skin while also stimulating collagen production over time. The result is a gradual improvement in skin firmness, texture, and fine crepiness. It is best suited for mild to moderate neck laxity and skin quality changes rather than significant loose skin.
PRF under-eye results commonly last 12 to 18 months, with repeat sessions building on previous collagen development. Sculptra results following a full treatment series commonly last two years or longer. Hyperdilute Radiesse for the neck commonly lasts 12 to 18 months. Individual longevity varies based on age, skin condition, lifestyle factors, and the number of sessions completed.
Yes, when appropriately selected.
PRF, Sculptra, and Hyperdilute Radiesse work in different anatomical areas and at different tissue depths, which is why they are commonly combined in treatment plans. The key is timing and sequencing. Some treatments are better performed before others depending on the plan, which is why a consultation is essential to determine the safest and most effective order for your specific goals.
Downtime is generally mild and temporary.
PRF under-eye injections may cause swelling or bruising for about 3 to 5 days due to the area’s sensitivity. Sculptra typically involves one to two days of mild swelling. Hyperdilute Radiesse for the neck may cause swelling or bruising for 2 to 4 days. Most patients return to normal daily activities quickly, especially compared to surgical procedures or more aggressive resurfacing treatments.
